This is list of the things we would ask be included in the contract. If you have your own version, that's fine. Your School Name and Address Contract Date Steve Ballard and Brett Garrett (If only one of us is working on your arrangement, the other's name will not be included on this contract.) herein referred to as Novapercussion Address Description of duties - Percussion score arrangement including Pit and Battery (all percussion) for fall marching band show - title or year - engaged by band boosters as an independent contractor OR engaged by school district as an employee of the county (stipulate terms) Term of Contract including any necessary "due date" Results and Responsibilities - Include any instructional or consultation duties here. Novapercussion agrees to perform said duties with consideration for discussed educational philosphy and purpose of (your school's) group. Compensation - List each duty with dollar amounts including payment plan and expected dates of payments. Describe source of payment. Termination - This contract may be terminated by employer for reasonable cause at any time. This contract may also be terminated by either party at any time upon receipt of thirty days' written notice. Upon such notification, no further payments will be made or expected. All payments already made to Novapercussion will be considered to have been earned and refund of payments to band boosters will not be expected. School regulations concerning this contract Interpretation of the agreement - this written contract incorporates all the rights and responsibilities between the employer (band boosters or school) and Novapercussion. Effective Date Signatures  We ask that payments be made by check. If the score will be written by only Steve or only Brett, please make out the checks to that person directly. If we are writing your arrangement in collaboration, the check may be written to either individually, it does not matter which.
